The standard
forms provided in this section have been
reviewed and approved by the City's Public
Works Department and City's legal counsel
for use and reliance by the City. Deviation
from the standard language provided herein,
other than language modified by the City,
shall require prior approval by the City.
Substantial deviations may, at the City's
direction, require review/approval of the
City's legal counsel and the fee for this
review may be billed to the customer.
Additional reviews may result in additional
time required for City review and approval
of a Certificate of Completion or
Certificate of Occupancy. Any forms which
require recording must bee accompanied by a
check from the customer equal to the amount
of all recording fees. |

The revision date appears at the bottom left corner of each form. Contact the City's Utilities Coordinator at 407.571.8340 to verify that you have the latest form.
EASEMENTS: All easement documents, exclusive of those recorded by plat, are to be submitted to the City Engineer or the City's Utilities Coordinator, as his designee, for review. This document must include an easement description accompanied by a sketch of description prepared on 8-1/2" x 11" paper at a legible scale suitable for recording. Text height shall be a minimum of one-tenth inch (1/10"). Descriptions and sketches must be prepared by a registered or professional land surveyor licensed in the State of Florida. All easement documents prepared by the surveyor must be signed and sealed.
Easement BONDS: All performance and maintenance bonds must be executed by a corporate surety company authorized to do business in the state of Florida. Bonds are to be submitted concurrently with final itemized costs of construction for review by the City Engineer for adequacy. Escrow agreement forms listed below cannot be used for projects with bond amounts less than $10,000 and require prior approval of the City Engineer, and are designated with an asterisk (*).
