House Check
House check forms may be downloaded and completed, however your signature must be witnessed by a records clerk or officer.
Completed forms may be returned to the Records Section of the Police Department between the hours of 7:30 AM and 4:30 PM, Monday through Friday.
You may also call (407) 339-2441 and an officer will be dispatched to your residence to witness your signature and retrieve the form.
